Standard clinical approaches do not always reflect the depth of immunological disorders. The role of innate immunity indicators, particularly NK-cells and interferons (IFN-β, IFN-γ, IFN-λ3, IFN-λ1), as biomarkers of disease severity has been studied only fragmentarily, which hinders the timely selection of adequate treatment strategies.</p> <p><bold><italic>AIM:</italic></bold><italic> </italic>To conduct a comparative analysis of certain indicators of innate immunity in patients with recurrent genital herpes in order to identify the most informative biomarkers determining the severity of the disease.</p> <p><bold><italic>METHODS:</italic></bold><italic> </italic>A cross-sectional single-center observational study was conducted, including 56 female patients with a confirmed diagnosis of recurrent genital herpes, based at the Yutskovskikh Professor’s Clinic, Vladivostok. The groups were formed according to the frequency of relapses per year (mild, moderate, and severe course). Laboratory examination included the analysis of cellular and cytokine components of innate immunity: serum concentrations of IFN-β, IFN-γ, IFN-λ3, and IFN-λ1 determined by enzyme-linked immunosorbent assay (ELISA), and NK-cell levels determined by flow cytometry. Statistical analysis was performed using the Shapiro-Wilk test; group comparisons were carried out using the Kruskal-Wallis test. Correlations were assessed using Spearman’s rank correlation coefficient, and the strength of associations was determined according to Chaddock’s scale.</p> <p><bold><italic>RESULTS:</italic></bold><italic> </italic>Analysis of the examination results of 56 patients distributed into groups showed that NK-cell count and IFN-β level were the key biomarkers of disease severity (<italic>p&lt;</italic>0.001 and <italic>p=</italic>0.001, respectively). A progressive twofold decrease in the median NK-cell count and more than a fourfold decrease in IFN-β levels were observed. It was established that IFN-γ levels decreased when comparing severe and mild disease courses (pIII–I=0.020), with the median reduced almost twofold. Analysis of IFN-λ3 and IFN-λ1 revealed no statistically significant differences (<italic>p=</italic>0.376 and <italic>p=</italic>0.412); however, complete absence of IFN-λ3 in serum was recorded in 39.3% of patients. A statistically significant direct weak correlation was found between NK-cells and IFN-β (ρ=0.265; <italic>p=</italic>0.049). The correlation between NK-cells and IFN-γ or IFN-λ3 was not statistically significant, while no correlation was found with IFN-λ1.</p> <p><bold><italic>CONCLUSION: </italic></bold>The study established that NK-cell count and IFN-β level are priority biomarkers, the stepwise decrease of which reliably determines the severity of the disease. The identified correlation between these indicators and the absence of IFN-λ3 production in 39.3% of patients confirms the combined nature of suppression of cellular innate and mucosal immunity with increasing severity of recurrent genital herpes. The study emphasizes the importance of further work within the framework of a larger-scale investigation.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Объективные критерии оценки тяжести рецидивирующего генитального герпеса остаются недостаточно разработанными. Стандартные клинические подходы не всегда отражают глубину иммунологических нарушений. Роль показателей врожденного иммунитета, в частности NK-клеток и интерферонов (ИФН-β, ИФН-γ, ИФН-λ3, ИФН-λ1), как биомаркеров степени тяжести заболевания изучена фрагментарно. Что препятствует своевременному выбору адекватной тактики лечения.</p> <p><bold>Цель исследования. </bold>Провести сравнительный анализ некоторых показателей врожденного иммунитета у пациенток с рецидивирующим генитальным герпесом для идентификации наиболее информативных биомаркеров, детерминирующих степень тяжести.</p> <p><bold>Методы. </bold>Проведено поперечное одноцентровое наблюдательное исследование, в которое были включены 56 пациенток с подтвержденным диагнозом рецидивирующий генитальный герпес, на базе Профессорской клиники Юцковских г. Владивостока. Формирование групп основано на частоте рецидивов в год (легкое, среднетяжелое и тяжелое лечение). Лабораторное исследование включало анализ клеточного и цитокинового звеньев врождённого иммунитета: концентрация ИФН-β, ИФН-γ ИФН-λ3 и ИФН-λ1 в сыворотке крови, определенные методом ИФА, содержание NK-клеток – методом проточной цитофлуориметрии. Статистический анализ, выполнен с применением критерием Шапиро–Уилка, сравнение групп – критерием Краскела–Уоллиса. Корреляционные связи оценивались с помощью коэффициента ранговой корреляции Спирмена, тесноту связи определяли по шкале Чеддока.</p> <p><bold>Результаты.</bold> Анализ результатов обследования 56 пациенток, распределённых по группам, показал, что содержание NK-клеток и уровень ИФН-β являются ключевыми биомаркерами тяжести процесса (<italic>p&lt;</italic>0,001 и, соответственно, <italic>p=</italic>0,001). Выявлено прогрессирующее снижение медианы в двое по NK-клеткам и более чем в 4 раза уровня ИФН-β. Установлено, что уровень ИФН-γ снижался при сравнении тяжелого течения с легким (<italic>pIII–I=</italic>0,020), медиана сократились почти вдвое. Анализ ИФН-λ3 и ИФН-λ1 статистически значимых различий не выявил (<italic>p=</italic>0,376 и <italic>p=</italic>0,412), однако у 39,3% пациенток зафиксировано полное отсутствие ИФН-λ3 в сыворотке крови. Выявлена статически значимая прямая связь слабой тесноты NK-клеток с ИФН-β (ρ=0,265; <italic>p=</italic>0,049). Корреляционная связь NK-клеток с ИФН-γ, ИФН-λ3 статически незначима, с ИФН-λ1 – связь отсутствует.</p> <p><bold>Заключение</bold>. В ходе исследования установлено, что содержание NK-клеток и уровень ИФН-β выступают приоритетными биомаркерами, ступенчатое снижение которых достоверно определяет степень тяжести процесса. Выявленная корреляция между этими показателями и отсутствие продукции ИФН-λ3 у 39,3% пациенток подтверждает сочетанный характер угнетения клеточного врожденного и мукозального иммунитета при увеличении степени тяжести рецидивирующего генитального герпеса.
